This Is Not About Writing Better Notes
If you're a PM, your problem is not “how to write documentation.”
Your problem is this:
- Exec says one thing in a quarterly meeting
- Team hears a slightly different version
- Delivery reflects something else entirely
And three weeks later, no one remembers what was actually decided.
That’s the real documentation problem.
AI didn’t create this problem.
It just made it worse — because now things move faster, decisions change faster, and misalignment compounds faster.
The Real Job: Managing Information Flow
As a PM, you don’t “write docs.”
You manage how information flows across 3 layers:
- Executive layer → strategy, direction, priorities
- Team layer → backlog, delivery, execution
- System layer → what actually gets shipped
If those 3 are not aligned, your product breaks — even if your code is perfect.
Most documentation fails because it only exists in one layer.
Where PMs Actually Live
Let’s be honest.
Most PMs don’t live in GitHub.
We live in:
- Google Docs / Google Drive
- Confluence
- Slack / Email
- Meeting notes
And that’s exactly why things fall apart.
Because these tools are:
- fragmented
- duplicated
- rarely maintained
You don’t have a documentation system.
You have scattered artifacts.
The Core Problem: Decision Drift
Here’s a pattern I see all the time:
- Decision made in a meeting
- Captured in notes (maybe)
- Slightly reinterpreted in a follow-up doc
- Further diluted in Jira / backlog
- Final implementation reflects assumptions
At no point do we validate:
“Is this still the original decision?”
That’s how you end up with:
- features that “technically correct” but wrong in intent
- stakeholders saying “this is not what I asked for”
- teams wasting cycles re-aligning
A Real Case: Consignee Authority vs Shipment Visibility
This is a more subtle (and more dangerous) case — because nothing is “obviously broken.”
Stakeholder discussion (business + compliance):
Decision direction: → Limit Consignee authority on booking actions (edit, change, SI submission) during Export stage
Reason: → Reduce risk of unauthorized changes
→ Align with compliance / ownership boundariesImplicit expectation: → Consignee should still have visibility into shipment status
Captured loosely as:
“Restrict consignee actions on export booking”
Engineering discussion:
Focus shifts to:
- role-based access control
- API permission handling
- UI enable/disable logic
Simplified interpretation: → “Consignee = restricted role during export stage”
What gets implemented:
Consignee cannot:
- edit booking
- submit SI
- trigger changes
But also:
- reduced visibility on shipment updates (unintended side effect)
After release:
Stakeholders ask: → “Why can’t consignee see shipment progress clearly?”
Engineering response: → “We restricted their role as discussed”
Business response: → “We said restrict actions, not visibility”
Everyone is technically correct.
But the outcome is wrong.
What actually broke?
Not the feature.
The interpretation between layers:
- “Authority restriction” became “role restriction”
- “Action control” became “access control”
- Visibility requirement was never explicitly documented
What I do differently now
For cases like this, I don’t allow a single-layer definition.
I force 3 aligned views:
1. Stakeholder intent
- Limit consignee actions
- Maintain necessary visibility
2. Constraint layer
- Compliance: restrict modification rights
- UX: visibility must not degrade
- Ops: avoid confusion for tracking
3. Engineering definition
- Disable specific actions (edit/change/SI)
- Keep read-only access to shipment milestones
- Ensure UI clearly separates “view vs act”
And before build, I validate:
Are we restricting behavior — or restricting access?
If that question is not explicitly answered, the implementation will drift.
What I Actually Do (As a PM)
1. Treat meeting notes as decision logs — not transcripts
Most meeting notes are useless.
They try to capture everything, so they capture nothing.
What matters is not:
- who said what
- how long the discussion took
What matters is:
- What was decided?
- What options were rejected?
- What assumptions were made?
If your notes don’t answer those 3 questions, they’re noise.
AI helps here:
- summarize discussions
- extract decisions
- structure messy conversations
But again — you still need to validate the output.
2. Create a single “broadcast version” for executive decisions
One of the biggest gaps I see:
Exec meetings happen → but the team never gets a clean version of it.
Instead:
- bits and pieces get shared in Slack
- PM paraphrases in standups
- everyone interprets differently
What I do instead:
After any important exec discussion, I create a broadcast version:
- 1 page max
- clear direction
- no ambiguity
- no meeting noise
This becomes:
- the reference for the team
- the anchor for backlog decisions
- the source of truth when conflicts happen
Without this step, alignment is just luck.
3. Separate “working docs” vs “source of truth”
Another common mistake:
Everything is treated equally.
But not all docs serve the same purpose.
I split them clearly:
Working docs (temporary)
- raw notes
- brainstorming
- drafts
→ messy by nature
Source of truth (stable)
- final decisions
- product direction
- release scope
→ clean, minimal, maintained
If you mix these two, your system collapses.
Because people don’t know what to trust.
4. Use AI to reduce friction — not replace thinking
Where AI actually helps me:
- turn meeting notes → structured decisions
- summarize long threads → single view
- draft updates for stakeholders
Where it does NOT help:
- deciding what matters
- validating alignment
- resolving ambiguity
If you rely on AI for those, you’re just scaling confusion faster.
Release Notes — Where Most Teams Fail Quietly
This is the most underrated part of PM documentation.
Most release notes are written like this:
- “Improved shipment visibility”
- “Enhanced permissions”
- “Bug fixes”
Which is basically useless.
Because when something breaks later, you can’t answer:
- What exactly changed?
- Why did we change it?
- What impact did we expect?
A better way to structure release notes
I keep it simple:
1. What changed → “Restricted consignee from booking actions (edit/change/SI submission) during export stage, while maintaining read-only shipment visibility”
2. Why it changed → “Ensure compliance and prevent unauthorized booking modifications”
3. Expected impact → “Reduce risk of incorrect changes while preserving transparency for consignee”
4. Risk / side effect → “Potential confusion if UI does not clearly differentiate restricted actions vs allowed visibility”
Why this matters
Because later, when something happens, you can:
- trace back decisions
- validate whether implementation matches intent
- identify whether issue is design vs execution
Without this, you’re guessing.
The Part Most PMs Avoid
Ownership.
Documentation breaks when:
- no one owns it
- or worse — everyone owns it
As a PM, you don’t need to own everything.
But you must own:
- product decisions
- stakeholder alignment
- release communication
If you don’t, the system will fragment — guaranteed.
The Reality
AI didn’t fix documentation.
It exposed that most PMs were never managing it properly in the first place.
The role is not “write docs.”
The role is:
make sure what was decided, what was communicated, and what was shipped are the same thing.
Everything else is secondary.
